1. Business Identity & Program Description
Envision Health MD (“we,” “our,” or “us”) may offer email notifications, sms notifications, mobile app push notifications, messaging services to provide customers with appointment confirmations, appointment reminders, rescheduling updates, customer support messages, and, where applicable, relevant service or promotional communications (only where explicit consent has been provided).
Messages may be sent to customers who have booked an appointment through our website at https://envisionhealthmd.com, scheduling forms, or other communication channels and have explicitly opted in to receive Email, SMS and/ or push notifications.
Opt-in is collected via web forms, scheduling forms, or other communication channels using a dedicated, unchecked checkbox that clearly provides consent to receive said messages.
Consent to Receive Messages
By providing your email and/ or phone number and opting in, you expressly consent to receive messages from Envision Health MD Consent and is not a condition of purchase.
2. Opt-Out & Support Mechanisms
You can cancel the notification services at any time. Just click on the unsubscribe link in any message. After you initiate your request to unsubscribe, we will email you a message to confirm that you have been unsubscribed. After this, you will no longer receive notification messages from us. If you want to join again, just sign up as you did the first time and we will start sending messages to you again. 3. Wireless Service Carrier Liability Clause
Wireless service carriers are not liable for delayed or undelivered messages.
4. Wireless Service Message Frequency & Fees
As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive messages periodically based on your interactions with our services. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
5. Supported Wireless Carriers
Our SMS program works with all major U.S. wireless carriers, including AT&T, T-Mobile, Verizon, Sprint, and most regional carriers.
6. Age Restriction
You must be 18 years or older to participate in our SMS program.

8. No Sharing of Mobile Information
